<p class="MsoNormal"><span style="font-family:"Arial",sans-serif">The Idaho Board of Nursing records indicate that you did not submit a 2020 renewal application for your LPN license. LPNs that did not complete the 2020 renewal application and attest to meeting
the renewal requirements resulted in their license expiring on August 31, 2020. As a result, you do not have an active Idaho LPN license to practice nursing effective September 1, 2020.

Requirements and a fingerprint-based background check. The reinstatement application may be accessed through the Nurse Portal at
