[Agencyitmanagers] Additional information needed for Outage notification SMS List

All,

A few weeks back I sent out the below email regarding an outage notification list we (Office of IT Services) are in the process of setting up.

We have learned that additional information is needed to set up the notifications.

If you have already emailed to request to be on our list (or if you haven't had a chance to, but would like to be added), please provide me with the following information (additional information not previously requested is highlighted).


  1.  Name
  2.  Agency
  3.  Email address
  4.  Business/desk phone number
  5.  Cell phone number (including area code)

(Cell phone must accept SMS messages)

  1.  Cell phone service provider (i.e. Verizon, AT&T, etc.)*

In an effort to improve critical communication to key staff, ITS is creating an outage notification SMS list in order to notify agencies in the event that email and/or telephone service is disrupted, and during critical events that may arise. If you or any of your staff would like to receive notifications via SMS on your cell phone, please email me at: Erin.Seaman at its.idaho.gov<mailto:Erin.Seaman at its.idaho.gov> with the following information:

  1.  Name
  2.  Agency
  3.  Cell phone number (including area code)

(Cell phone must accept SMS messages)
