They're now statutory forms.  There's the health care POA and a general POA.
They're really pretty good.  § 15-12-301. STATUTORY FORM POWER OF ATTORNEY
is the general form and the advanced directive for health care is in §
39-4510. LIVING WILL AND DURABLE POWER OF ATTORNEY FOR HEALTH CARE.  The
latter statute says that "such document SHALL BE in substantially the
following form . . ."  The one in 15-12-301 carries some automatic
provisions that your client will probably want.  I dumped my prior forms
when these came out.

make sure her brother can make financial and medical decisions for her
should anything happen. Anyone have a power of attorney form that I could
