[CLBS] Declaration/affidavit re cram down - any samples?

Hello, the court wants an Affidavit or Declaration providing the Court with
information that supports the confirmation of the Amended Plan (specifically
with respect to the cram down of one creditor).  Most of my argument is
already laid out in my pre-confirmation memo, but I am wondering whether
anyone has a sample of what Judge Pappas is looking for in this type of
declaration so I do not have to reinvent the wheel.
