[CLBS] Form and Briefing Request

I haven’t yet had to file a motion for contempt for violation of a discharge injunction against a no-asset Chapter 7 client’s alleged creditor, but the time is ripe and I will be filing against the alleged creditor’s counsel also.  Does anybody have some sample forms, briefing or pointers for me?
