[CLBS] Ch 11 sm biz combined plan/disclosure statement - samples, do I need a court order?

I have a small business chapter 11 case. I want to do a combined plan and
disclosure statement and serve it. Do I need court approval (separate order)
in order to do this? Can someone send me a sample of theirs? Looking to file
Friday so time is of the essence. Thanks.
