[CLBS] Chapter 7 522(f) motion

Does anyone have a sample 522(f) motion to remove a judgment lien from real
property? I am specifically looking for what the courts here need as
evidence of value, etc. In CA, we have to have a recent appraisal, certified
copies of the judgment, etc. What do you need here and can you point me to a
sample case where one was granted?
