[CLBS] Medicaid & Medicare & liens

I had a consultation with a debtor in CA, age 73, who transferred a house to
her son 2 years ago for zero dollars (bankruptcy not advised). She's been on
Medicaid and Medicare for many years. She asked me whether Medicare or
Medicaid would have a lien on the house she transferred to her son. How do
such liens arise, and are they recorded so that I could find out with a
preliminary title report?
