[CLBS] Power of attorney samples that banks will accept

Does anyone here have a sample "power of attorney" that you send to banks so
they will talk to you if you haven't filed a BK for the client? I'm getting
the usual runaround. Multiple "authorizations" have been sent to the
bank(s), all lost or not good enough. Client tells the bank to call me. They
call me, then won't talk to me because there is no "power of attorney" on
file. I'm dealing with 5 banks for this client. Thanks.
