[Icoord] CYBER LIABILITY PII-PHI COUNTS

Good morning Insurance Coordinators,

The State's Cyber Liability insurance policy will expire July 1, 2020.  For my office to renew this policy, the insurance underwriter from Lloyd's/Brit is not requiring new applications this year, however have requested updated 2020 PII/PHI count (personally identifiable information or personal health information) involving your specific agency.  You may have to coordinate with your IT folks to obtain the information required on the attached template.

As further clarification when completing the attached template, for example, if you have employees or other individual's names listed multiple times in your specific data base with any kind of PII/PHI information about them, ONLY COUNT the name as a record one time, NOT multiple times.

Please complete and return the attached template to me prior to February 28, 2020.
