[CLBS] Medicaid liens - substantial compliance

Has anyone litigated whether the state has substantial complied with I.C.
56-218A and I.C. 45-1904 (Medicaid liens) when it (1) fails to list the
debtor's address on the lien and (2) fails to list a dollar amount for the
lien? I know there are no reported decisions on point for either of those
statutes but I was wondering if anyone had dealt with the issues and
resolved it short of a written decision.
