[IDHR] Donated Leave Request

Good afternoon,

We have two employees that need donated leave due to a qualifying reason. We have reached out to our agencies that we support already.
If you are able and willing to assist an employee, please email me with the number of hours you would like to donate. Once I receive your email with the hours you wish to donate, I will send you the form for you to sign.

We would greatly appreciate your donation!

Please make sure you meet the qualifications below and let me know how many hours you would like to donate as well as if is it sick or vacation time.


  *   You must donate a minimum of 4 hours.
  *   You may donate either Sick or Vacation leave as long as you have 80 hours of the type you donate remaining after donation.
  *   You cannot donate more than a maximum of 80 hours of vacation or sick per fiscal year.
  *   The donated time converts to sick leave for the receiving employee.

Please let me know as soon as possible, but by September 28th at noon at the latest, if you would like to donate.
